Sometimes it can be hard to take care of your health. If you need help, please call us.CareFirst Community Health Plan Maryland (CareFirst CHPMD) offers Case Management to members who have had a critical medical event, been diagnosed with a chronic illness, and have multiple illnesses or complications.
✓ We will work with you and your doctors to help you get the services, resources and one-one coaching you may need to better take care of your health.
✓ You will get your own CareFirst CHPMD Case Manager. Your Case Manager will:
- Help you set goals and make a care plan to reach them
- Make sure you get the care you need when you need it
- Make sure you have what you need to get care, such as a way to get to and from the doctor
- Find ways to help you manage your symptoms so you can feel better
- Answer questions about your health and health care
- Keep you up to date with information about your health
- Answer any questions you may have about your health over the phone. If you want us to visit you in your home or at your doctor’s office, we can do that too.

This will ensure the healthiest pregnancy possible. Your first prenatal visit should be in your first trimester of pregnancy or within 42 days of enrolling with CareFirst CHPMD.
